if not from network profile or a profile, is the phone registered with
MDM server - eg corporate email (outlook) etc in which case this message
may occur. also, reports seem to suggest that its a random bug that can
occasionally be triggered by accident - some codepath gets visited when
it shouldnt (unless you have a profile asking for strict passcode or are
enrolled
with MDM)
